| Abstract: | Das Zytomegalievirus, auch Humanes Herpesvirus 5, gehört zur Familie der Herpesviren und persistiert
nach der Primärinfektion lebenslang im Körper. Bei immunsupprimierten Patienten, insbesondere nach
allogener Stammzell- oder Organtransplantation, stellen CMV-Reaktivierungen eine wesentliche
Ursache für Morbidität und Mortalität dar. Die etablierten antiviralen Medikamente Ganciclovir und
Valganciclovir sind in ihrem therapeutischen Erfolg durch Resistenzentwicklungen, Myelosuppression
und ein komplexes Nebenwirkungsprofil limitiert.
Ziel dieser Arbeit war es, das diagnostische und therapeutische Potenzial eines HLA-restringierten,
Zytomegalievirus-spezifischen Antikörperrepertoires in Patientenproben zu ermitteln. In
vorangegangenen Projekten wurden mithilfe der Phage-Display-Technologie Antikörperfragmente
(Fabs) generiert, die Zytomegalievirus-Peptide erkennen, welche im Komplex mit den MHC-Klasse-I-
Molekülen HLA-A*0101, HLA-A*0201 und HLA-B*0702 auf der Zelloberfläche präsentiert werden.
Untersucht wurden die drei repräsentativen Antikörper A6 (HLA-A*0101), C1 (HLA-A*0201) und C7
(HLA-B*0702).
Für die diagnostische Anwendung wurden die Antikörper im Fab-Format als Tetramere eingesetzt und
in frischen Blutproben von Zytomegalievirus-infizierten Patienten sowie gesunden Kontrollpersonen
mittels Durchflusszytometrie getestet. Die Tetramere zeigten eine spezifische Bindung an Lymphozyten
von Patienten mit CMV-Infektion und passendem HLA-Status, während in Kontrollproben (passender
HLA-Status und keine Infektion oder CMV-Infektion aber nicht passender HLA-Status) keine Bindung
nachweisbar war. Longitudinale Messungen zeigten eine Abnahme der Affinität der Tetramere parallel
zur Reduktion der Viruslast und verdeutlichen damit das Potenzial für deren diagnostischen Einsatz.
Das therapeutische Potenzial wurde in Zytotoxizitäts-Assays mit Fc-optimierten IgG-Vollantikörpern
überprüft. Hierbei induzierten die Antikörper C1 (HLA-A*0201) und C7 (HLA-B*0702) in peripheren
mononukleären Zellen von Zytomegalievirus-positiven, HLA-kompatiblen Patienten eine spezifische
Lyse infizierter Zellen. In Zytomegalievirus-negativen, HLA-kompatiblen Kontrollproben war eine
Lyse nur nach kombinierter Zugabe von Zytomegalievirus-Peptid und Antikörper nachweisbar, während
bei einem Zytomegalievirus-positiven Patienten ohne passendes HLA-Allel keine spezifische Lyse
beobachtet werden konnte.
TCR-ähnliche Antikörper können also sowohl diagnostisch zur Identifikation Zytomegalievirus-
infizierter Zellen als auch therapeutisch zur gezielten Elimination dieser Zellen eingesetzt werden.
Durch die Entwicklung eines HLA-diversen Antikörperrepertoires ließe sich die klinische
Anwendbarkeit potenziell erweitern. Damit bieten TCR-ähnliche Antikörper einen innovativen Ansatz zur Prävention und Behandlung Zytomegalievirus-assoziierter Komplikationen bei
immunsupprimierten Patienten. Human cytomegalovirus, also known as human herpesvirus 5, belongs to the family of herpesviruses and persists lifelong in the host after primary infection. In immunosuppressed patients, particularly following allogeneic stem cell or solid organ transplantation, reactivations of cytomegalovirus represent a major cause of morbidity and mortality. The established antiviral drugs ganciclovir and valganciclovir are limited in their therapeutic efficacy by the emergence of resistant strains, myelosuppression, and a complex profile of adverse effects. The aim of this study was to investigate the diagnostic and therapeutic potential of an HLA-restricted, cytomegalovirus-specific antibody repertoire in patient samples. In preceding projects, antibody fragments (Fabs) were generated by phage display technology that recognize cytomegalovirus-derived peptides presented in complex with the MHC class I molecules HLA-A*0101, HLA-A*0201 and HLA- B*0702. Three representative antibodies were analyzed: A6 (HLA-A*0101), C1 (HLA-A*0201), and C7 (HLA-B*0702). For diagnostic evaluation, the antibodies were applied in the Fab format as tetramers and tested by flow cytometry on fresh blood samples from cytomegalovirus-infected patients and healthy controls. The tetramers specifically bound to lymphocytes of CMV-infected patients carrying the respective HLA allele, while no binding was observed in control samples. Longitudinal measurements demonstrated a decrease in tetramer binding in parallel with the reduction of viral load, thereby highlighting the potential of Fab tetramers for monitoring cytomegalovirus reactivation. Therapeutic potential was assessed in cytotoxicity assays using Fc-optimized full-length IgG antibodies. In these experiments, the antibodies C1 (HLA-A*0201) and C7 (HLA-B*0702) induced specific, HLA- dependent lysis of infected target cells in peripheral blood mononuclear cells from cytomegalovirus- positive, HLA-compatible patients. In cytomegalovirus-negative, HLA-compatible controls, lysis was only observed after combined addition of peptide and antibody, whereas no specific lysis was detected in a cytomegalovirus-positive patient lacking the corresponding HLA allele. These findings demonstrate that T-cell receptor-like antibodies could be employed both diagnostically for the identification of cytomegalovirus-infected cells and therapeutically for their targeted elimination. The development of a modular, HLA-diverse antibody repertoire may further broaden clinical applicability. Thus, T-cell receptor-like antibodies indicate a promising novel approach for the prevention and treatment of cytomegalovirus-associated complications in immunosuppressed patients. |
