Abstract: | Der hyaline Gelenkknorpel und der
subchondrale Knochen stehen als
osteochondrale Einheit in enger
Wechselbeziehung zueinander. Gelenkknorpelschäden, Meniskus- und
Ligamentverletzungen sind bei Spor tlern oft mit Knochenmarködemen
assoziiert.
Dieser Übersichtsartikel beschreibt
die Ursachen von subchondralen
Knochenmarködemen mit einem Fokus auf deren klinische Relevanz im
Sport. Sowohl konservative als auch
operative Behandlungsansätze werden dargestellt.
Knochenmarködeme treten beim Athleten relativ häufig auf. Sie sind
meist als belastungsinduziert zu werten, finden sich aber auch posttraumatisch, postoperativ oder im
Rahmen von Insuffizienzfrakturen
und Osteonekrosen. Die klinische
Relevanz von Knochenmarködemen
ist auch beim Sportler noch unklar.
Es existieren unterschiedliche konservative als auch operative Therapieoptionen zur Behandlung
symptomatischer Knochenmarkö -
deme, die stufenweise aufeinander
aufbauen.
Ödemen des subchondralen Knochens
fällt eine entscheidende Rolle für die
Gelenkfunktion und -integrität sowie
die Rekonvaleszenz nach Gelenkverletzungen zu. Articular cartilage and subchondral bone are intimately connected within the osteochondral unit. Articular cartilage defects, meniscus and liga ment injuries are often associated with subchondral bone edema in sports. This review summarizes the etiology, clinical relevance and treatment op tions for bone marrow edema in athletes. Subchondral bone marrow edema are frequently observed in athletes and most commonly induced by mechani cal overload. Yet, such edema have also been observed posttraumatically, postoperatively or in association with stress fractures and osteonecrosis. Their clinical relevance in athletes still remains unclear. Non-surgical as well as surgical treatment options exist for symptomatic bone marrow edema. Bone marrow edema play an impor tant role in the function and integrity of joints and are associated with the clinical outcome following joint injuries. |
