| Abstract: | Fragestellung: Die Rhabdomyolyse ist ein potenziell lebensbedrohliches Syndrom mit Freisetzung von Myoglobin (Mb), das durch direkte Tubulusobstruktion und -schädigung sowie sekundärer renaler Vasokonstriktion eine akute Nierenschädigung verursachen kann. Vereinzelte Fallberichte deuten darauf hin, dass der Einsatz des CytoSorb-Adsorbers (CS, CytoSorbents Europe GmbH, Berlin) die Myoglobinelimination beschleunigen kann, randomisierte kontrollierte Studien fehlen jedoch. In dieser randomisierten kontrollierten Pilotstudie wurde untersucht, ob die Integration von CytoSorb in eine kontinuierliche venovenöse Hämodialyse (CVVHD) die Myolgobinelimination beschleunigt.
Material, Methoden: Insgesamt wurden 8 Patienten randomisiert und in die Studiengruppen CS und Kontrolle (K) zugeteilt. Die Einschlusskriterien umfassten Alter ≥ 18 Jahre, eine Indikation zur CVVHD, Serum-Mb > 30.000 µg/l oder > 10.000 µg/l bei gleichzeitig eingeschränkter glomerulärer Filtrationsrate. Die Kontrollgruppe (K) erhielt über 48h eine CVVHD mit einem großporigen Hämofilter (EMiC-2, Fresenius Medical Care DE GmbH, Bad Homburg) unter Verwendung hoher Flussraten (Blutfluss 200 ml/min, Dialysatfluss 4 l/h). In der Gruppe CS erfolgte die Behandlung mit dem gleichen Hämofilter und unter gleichen Flussraten, ergänzt durch einen vor den Hämofilter integrierten CytoSorb Adsorber, der nach 24 Stunden gewechselt wurde. Blutproben wurden 5 min, 30 min, 2h, 4h, 8h und 24h nach Beginn der Behandlung jeweils vor und nach CS bzw. vor und nach dem EMiC-2-Filter entnommen (Mbprä, Mbpost). Die Messungen wurden am zweiten Studientag wiederholt. Die primäre Zielgröße war die Area under the Curve (AUC) des prozentualen Mb-Abfalls über die ersten 24h. Zudem wurde die relative Mb-Reduktion (RR) durch CS bzw. durch den EMiC-2-Filter zu den jeweiligen Messzeitpunkten bestimmt (RR = (Mbprä - Mbpost)/Mbprä*100%). Die Daten beider Studientage wurden für die Analyse zusammengefasst. Sowohl die primäre Zielgröße als auch die relativen Reduktionswerte wurden zwischen den Studiengruppen mit dem Mann-Whitney-U-Test für unabhängige, nicht normalverteilte Stichproben verglichen.
Ergebnisse: 4 Patienten wurden in die Gruppe CS, 4 in die Gruppe K randomisiert. Die AUC des prozentualen Mb-Abfalls über 24 Stunden war in der Gruppe CS mit 42 10% signifikant kleiner als in der Gruppe K mit 63 6% [Mittelwert SD] (p = 0,029). Die RR des CS war in den ersten 8 h zu allen Zeitpunkten deutlich signifikant größer als die des EMiC-2-Filters.
Schlussfolgerung: Der Einsatz des Hämadsorbers CS führte innerhalb von 48 Stunden zu einem signifikant stärkeren und deutlicheren Abfall der Myoglobinkonzentration. Durch einen Sättigungseffekt nimmt die Eliminationsleistung von CS im Zeitverlauf ab, weshalb kürzere Wechselintervalle, z.B. alle 8-12 Stunden erwogen werden sollten. Introduction: Rhabdomyolysis is a potentially life-threatening syndrome associated with the release of myoglobin (mb), which can cause acute kidney injury through direct tubular obstruction and damage as well as secondary renal vasoconstriction. Isolated case reports suggest that the use of the CytoSorb adsorber (CS, CytoSorbents Europe GmbH, Berlin) may accelerate myoglobin elimination, however randomized controlled studies are lacking. This randomized controlled pilot study investigated whether integration of CytoSorb into continuous veno-venous hemodialysis (CVVHD) accelerates myoglobin elimination. Methods: A total of 8 patients were randomized into the CS and control (K) study groups. Inclusion criteria were age ≥ 18 years, an indication for CVVHD, and serum myoglobin > 30,000 μg/l or > 10,000 μg/l in the presence of impaired glomerular filtration rate. The control group received CVVHD with a high cut-off hemofilter (EMiC-2, Fresenius Medical Care Germany GmbH, Bad Homburg) using moderately high flows (blood flow: 200 ml/min, dialysate 4 l/h). In the CS group, treatment was performed with the same hemofilter and identical flow rates, supplemented by the integration of a CytoSorb adsorber upstream of the hemofilter, which was replaced after 24 hours. Blood samples were collected at 5 min, 30 min, 2h, 4h, 8h and 24h after initiation, both before (pre) and after (post) the hemoadsorber in the CytoSorb group, and before and after the hemofilter in the control group. Measurements were repeated on the second study day. The primary outcome was the area under the curve (AUC) of percent mb over the first 24h. Additionally, relative myoglobin reductions were calculated for both CytoSorb and EMiC-2 ((mbpre - mbpost)/mbpre*100%). Data from both study days were pooled for analysis. The primary outcome and the relative reductions were compared between study groups using the Mann-Whitney U test for independent, non-normally distributed samples. Results: 4 patients were randomized to group CytoSorb, 4 to the control group. The AUC of the Mbpre concentrations as percent of baseline during the first 24 hours was significantly lower in the CytoSorb group with 42 10% compared to the control group with 63 6% mean SD (p = 0.029). The relative myoglobin reductions of the CytoSorb-adsorber were significantly higher than those of the EMiC-2-filter during the first 8 hours. Conclusion: Use of the CytoSorb hemoadsorber resulted in a significantly stronger and more pronounced reduction in myoglobin concentrations within 48 hours. Due to a saturation effect, the elimination capacity of CS decreases over time, therefore, shorter replacement intervals, for example every 8-12 hours, should be considered. |
